WHAT IS MEDICARE?

Understanding Medicare: Parts A, B, C & D

Original Medicare is the federal health insurance program made up of Part A (Hospital Insurance) and Part B (Medical Insurance). Part C (Medicare Advantage) is a private-plan alternative for receiving Part A and Part B benefits, while Part D provides prescription drug coverage through private plans.

MEDICARE PART A
(HOSPITAL INSURANCE)

What it helps cover: Inpatient hospital care, skilled nursing facility care, hospice care, inpatient rehabilitation, and some home health care.

2026 premium: $0 per month for most people who have enough Medicare-covered work history (generally at least 40 quarters/10 years). If you do not qualify for premium-free Part A, the 2026 premium may be $311 or $565 per month, depending on work history.

2026 inpatient hospital deductible: $1,736 per benefit period. After the deductible, hospital coinsurance is $0 for days 1–60, $434 per day for days 61–90, and $868 per lifetime reserve day for days 91–150. Other cost-sharing rules may apply.

MEDICARE PART B
(MEDICAL INSURANCE)

What it helps cover: Doctor and other health care provider services, outpatient care, durable medical equipment, certain home health services, preventive services, and other medically necessary services covered by Medicare.

2026 standard premium: $202.90 per month. Some beneficiaries pay more because of income. Refer to the IRMAA charts below.

2026 annual deductible: $283. After the deductible, you generally pay 20% of the Medicare-approved amount for many Part B-covered services when the provider accepts assignment.

MEDICARE PART C
(MEDICARE ADVANTAGE)

What it is: Medicare Advantage is another way to receive your Medicare Part A and Part B benefits through a private insurance company approved by Medicare. You must have both Part A and Part B to enroll and generally continue paying your Part B premium.

What it may include: Many plans include Part D prescription drug coverage and may offer additional benefits such as dental, vision, hearing, fitness, transportation, or over-the-counter allowances.

Important: Premiums, provider networks, copays, coinsurance, prior authorization requirements, benefits, and maximum out-of-pocket limits vary by plan and service area.

MEDICARE PART D
(PRESCRIPTION DRUG COVERAGE)

What it helps cover: Prescription medications through a standalone Medicare drug plan or through a Medicare Advantage plan that includes drug coverage.

How plans differ: Each plan has its own covered-drug list, pharmacy network, premiums, deductible, copays, coinsurance, and coverage rules. Costs can depend on the medications you take and the pharmacy you use.

2026 out-of-pocket limit: After your out-of-pocket spending on covered Part D drugs reaches $2,100 in 2026, you enter catastrophic coverage and pay $0 for covered Part D drugs for the rest of the calendar year. Plan premiums and medications not covered by your plan do not count toward this limit.

Important: A late-enrollment penalty may apply if you go without Part D or other creditable prescription drug coverage for 63 consecutive days or more after your Initial Enrollment Period.

Who qualifies for Medicare?

Medicare is generally available to people age 65 or older, certain younger people with qualifying disabilities, and people with End-Stage Renal Disease (ESRD). Special rules also apply for people with ALS and certain other circumstances.

Some people are automatically enrolled in Part A and Part B, while others need to sign up. If you are still working or covered through an employer plan when you become eligible, the timing of Part B enrollment can be especially important.

Important enrollment considerations

Waiting to enroll when you are first eligible can result in late-enrollment penalties unless you qualify for a Special Enrollment Period. For Part B, the late-enrollment penalty is generally an additional 10% of the standard premium for each full 12-month period you could have had Part B but did not enroll, and the penalty may continue for as long as you have Part B.

What Original Medicare does not fully cover

Original Medicare has deductibles and coinsurance and generally has no annual out-of-pocket maximum. Many beneficiaries consider a Medicare Supplement (Medigap) policy, Medicare Advantage plan, and/or prescription drug coverage depending on their needs and eligibility.

Medicare and Medicaid: Can You Have Both?

Yes. Some people qualify for both Medicare and Medicaid. Eligibility and the help available depend on income, resources, household circumstances and Oklahoma program rules.

Medicare and Medicaid Are Different

Medicare is federal health coverage generally based on age, disability or certain medical conditions. Medicaid is jointly funded by federal and state governments and provides coverage to people who meet applicable financial and other eligibility requirements. A person who qualifies for both is often called dual eligible.

OKLAHOMA SOONERCARE SUPPLEMENTAL

Medicare Savings Programs

Depending on eligibility, an Oklahoma Medicare Savings Programβ€”also called SoonerCare Supplementalβ€”may help pay the Medicare Part B premium and, in some categories, other Medicare cost sharing such as deductibles and coinsurance. People should apply even if they are unsure whether they qualify.

MEDICARE SAVINGS PROGRAMS

Extra Help and D-SNP Options

People who qualify for Medicaid or certain Medicare Savings Programs may automatically qualify for Extra Help with Medicare Part D prescription costs. Some dual-eligible beneficiaries may also qualify for a Dual-Eligible Special Needs Plan (D-SNP), where available. Networks, prescriptions, benefits and eligibility should be reviewed carefully.

Program limits and eligibility rules can change. Great Nation Insurance does not determine Medicaid, Medicare Savings Program or Extra Help eligibility. Applications and final eligibility decisions are handled by the appropriate government agency.

2026 Medicare IRMAA Charts

IRMAA (Income-Related Monthly Adjustment Amount) is an additional Medicare charge that may apply when your modified adjusted gross income is above certain levels. The 2026 amounts below are based on the official CMS 2026 Medicare premium figures.

Married filing separately: Different IRMAA thresholds apply if you were married and lived with your spouse at any time during the tax year but filed a separate return. IMPORTANT MEDICARE DATES

Understanding Medicare Enrollment Periods

Medicare uses different enrollment periods for signing up for Part A and Part B and for joining or changing Medicare Advantage and Part D plans. Knowing which period applies can help you avoid coverage delays and possible late-enrollment penalties.

Signing Up for Medicare Part A and Part B

Initial Enrollment Period

A 7-month period around age 65

For most people, this period begins three months before the month they turn 65, includes their birthday month, and continues for three months afterward. Different timing can apply when a birthday falls on the first day of a month.

General Enrollment Period

January 1–March 31

This period may be available if you did not enroll in Part A or Part B when first eligible and do not qualify for a Special Enrollment Period. Coverage generally starts the month after you sign up, and late-enrollment penalties may apply.

Special Enrollment Period

Timing depends on the qualifying situation

People covered by qualifying current-employment group health coverage may be able to delay Part B and enroll while that coverage continues or during an eight-month period after the employment or coverage ends, whichever happens first. COBRA does not extend this Part B deadline.

Joining or Changing Medicare Advantage and Part D Coverage

Initial Plan Enrollment

When you are new to Medicare

Your opportunity to join a Medicare Advantage or Part D plan is generally connected to when you first become eligible for Medicare. Medicare Advantage enrollment requires both Part A and Part B.

Medicare Open Enrollment

October 15–December 7

You can join, drop, or switch Medicare Advantage or Part D coverage and may change between Original Medicare and Medicare Advantage. Changes generally take effect January 1 of the following year.

Medicare Advantage Open Enrollment

January 1–March 31

If you are already enrolled in Medicare Advantage, you can generally switch to another Medicare Advantage plan or return to Original Medicare and, when applicable, join a separate Part D plan. This period does not allow someone with Original Medicare to join Medicare Advantage.

Other Special Enrollment Periods may apply. Certain eventsβ€”such as moving, losing or changing coverage, qualifying for Medicaid, or receiving Extra Helpβ€”may create another opportunity to join or change a Medicare health or drug plan. Eligibility, allowed changes, and deadlines depend on the specific situation.

Why you may need it: A sewer or drain backup can damage flooring, walls, furniture, appliances, and other belongings, and cleanup can be expensive. Standard homeowners policies may exclude or limit this type of loss unless water backup coverage is specifically added.

Important distinction: Water backup coverage is not the same as flood insurance. It generally does not cover rising surface water or other flooding that requires a separate flood policy.

Optional Endorsements

What they cover: Endorsements can modify or add protection for needs not fully addressed by a standard policy, depending on the insurerβ€”for example, certain valuables, equipment breakdown, water backup, or other exposures.

Why you may need them: Standard homeowners policies have limitations and exclusions, so additional protection may be appropriate for your property and belongings.
